Enter a Skilled Visit Record (Mobile App)

It is best practice (and required by the Minnesota DHS EVV Mandate) to have your Skilled Caregiver enter the Visit record electronically. To do this, the Client must have an active Skilled Care Delivery and Service Agreement, and they must be assigned as a Caregiver on the Client’s Care Team.

To enter a Skilled Visit record:

1. Open the Pavillio Caregiver Mobile App.

2. Go to My Clients and locate the Client.

3. Tap on Real Time Clock In.

4. Select the Service Line.

5. Select the Service Agreement and Service Code.

6. Select the correct Document Template.

7. Answer the questions.

8. Enter the Notes.

9. Tap Submit.

10. You can also tap Save to submit the record at a later date. To find your saved record, go to the Dashboard and tap Visits to Submit. Tap Edit to finish filling out the Visit information, and then tap Submit.
