Masters Forms Feature
The Forms feature is under Administration > Masters > Forms. This is a list of forms. This area is managed by Pavillio. You can search forms but cannot add, edit, or delete Forms.

The Agency Admin needs to activate the OASIS form so that it is available for use in the Client profiles. You need to enter your HHA Agency ID provided to you by CMS.

To activate OASIS forms for your Agency:

Go to Administration > Masters > Forms.

In the OASIS form's row, click the Ellipsis under the Actions column and select Activate.

Go to Administration > Agency > Agency Information. Click the Edit button at the top of the page.

In the UMPI/NPI & FEIN Information section, find the UMPI/NPI in the table and click the Ellipsis under the Actions column. Select Edit.

Enter your Agency's CMS-provided HHA Agency ID that matches the UMPI/NPI.

Click the Update button.

Click the Update button at the top of the page.
