Request for Information (RFI)

When setting up Pavillio Payroll, If your business can’t be verified automatically, you’ll receive a Request for Information (RFI). This request asks you to provide an IRS document that includes your legal business name, Federal Employer Identification Number (FEIN), and filing address.

The most commonly used verification document is the CP-575. A CP-575 is an IRS notice that confirms the business has been issued a FEIN. The notice includes important tax information about the business including:

  • FEIN, a nine-digit number used for conducting business

  • Legal business name and filing address

  • Federal tax forms the company is required to file

There are two ways to provide a RFI document: during Onboarding or in the Dashboard.

When an RFI is required during Onboarding, an additional step called Verify your business details populates in the onboarding page. Select Start to upload the RFI document.

When an RFI is required after Onboarding is completed, the required document can be uploaded by navigating to the Dashboard then selecting Start next to Additional documentation is required.

After the upload is complete, our team reviews the document and follows up if any additional information is required. Once completed, your account status is updated to Ready.

If you’ve misplaced your CP-575 letter, a FEIN Verification Letter (147C) can be requested by following the below instructions:

  1. Before calling, ensure you have access to a fax to receive the document ASAP, otherwise it can take 4-6 weeks to get it in the mail. If you don’t have a fax, you can use efax.com, myfax.com, or hellofax.com to obtain a fax number.

  2. Call the IRS Business & Specialty Tax line at 800-829-4933

  3. Press 1 > Press 1 > Press 3

  4. Once the agent is on the line, inform them that you lost your CP-575 and need a FEIN Verification Letter (147C).

  5. The agent asks you a few security questions, and if you want to receive the 147C by secure fax or mail. You’ll receive it instantly if you ask for it by fax. Answer YES when the agent asks if you have a private and secure fax next to you.
