Add Client Goals and Outcomes

Goals and Outcomes help you track the client’s progression towards desired goals as part of Care Delivery.

You must have a Care Period and Service Agreement in place before setting up Goals and Outcomes.

To add Goals and Outcomes:

  1. Go to Clients > Current Clients and find the Client record.
  2. Go to the Care tab.
  3. Find the appropriate Care Period and click the Add Care Delivery icon under the Actions column.

  4. The Select Document Template window opens. Select the appropriate plan of care from the list and click Select.

  5. On the following page, enter a Start Date, End Date, and Notes. Fill out any additional information. Click Save.

  6. Click the Add/Edit Outcomes button.

  7. The Outcomes window opens. Fill out the form, adding as many Outcomes as needed. Outcomes are added one at a time.

  8. Select Measurement Type - Rating Scale:

    1. Count (Above The Threshold) – (0-10)

    2. Rating Scale (Simple Average) – (0-10)

    3. Time To Complete (in minutes) – (0-999999)

    4. Yes/No (% of total) – (Yes, No.)

  9. Enter the Positive Qualifier.

  10. Enter the Target Goal (%).

  11. Select Staff Responsible. To appear on this menu, people must be assigned the Role of Agency Case Manager.

  12. Add a Start Date. You will add an End Date when the goal is completed.

  13. Enter an Outcome Description. Staff can read this on the Pavillio Mobile App and under My Clients when completing visit documentation.

  14. Add Outcome Methodology. Staff can read this on the Pavillio Mobile App and under My Clients when completing visit documentation.

  15. Answer the remaining questions, adding notes as needed.

    1. Criteria for achievement.

    2. Identify changes or modifications to the physical and social environment necessary.

    3. Identify required equipment and materials.

    4. Identify the person’s communication style.

    5. Identify the Client’s learning style.

  16. Click Add.

  17. Repeat as needed for additional goals.

  18. Click Save.
